This present study is carried out for the evaluation & standardization of the “Dark Adaptometer” & to develop an improved screening method as well as assessing method for the measurement of vitamin A level

the Instrument works on the the “dark adaptation” of human eye, which states that “the adjustment of the eye to low light intensities” the light will be measured in 0 to 900 absolute Units, the evaluation for this instrument will be measured by assessing the random volunteers & vitamin A deficient patients, after measuring with “dark Adaptometer” the patients will be given a single vitamin A prophylaxis and again with the  “dark adaptometer” the improvement in vision will be assessed.

the serum vitamin A level in patients will be carried out according to accessible facilities.

the following instrument works on the principle “Dark Adaptation” of human eye which states “adjustment of the eye to low light intensities” the light will be measured in 0 to 900 Absolute units(this will measure Indirect vitamin A level)

present study will be carried out in 200 subjects(100 from upper socio-economic class from Rural area + 100 from poor socio economic class from Rural area), the subjects will be tested with “Dark Adaptometer” to measure the unit of light required , The Subjects who requires light >600 absolute units will be given Vitamin A supplement ( 2,00,000 IU) ,After 1 week again they will be tested with “Dark Adaptometer” for assessment of improvement, The serum vitamin A level will be carried out for conformational evaluation

Technical details

To be able to detect the defect in eye sight, we created a controlled environment in terms of managing visible light intensity within an instrument.   With the help of microcontroller and special programming, a circuit has been developed which is able to return extremely precise results over a large period of time. Components used are very durable and are having exceptionally long life span, such as power LED, 7 segment LED display, AVR 8 bit microcontroller etc. Sensitive components such as Tactical Switch, are protected by a specially designed casing, to save them from environmental damage. The brightness of power LED – responsible for all the lighting inside the machine, is easily and precisely controlled between 0 to 999 units. This unit can be converted to Lumens for increased reliability if required. There are two tactical switches, accessible to operator, to increase or decrease the intensity of light present, with a great extent of smoothness. To meet requirement of dynamic conditions, a feature of fast change in brightness is also included which is triggered automatically when a switch is active for more than a few seconds. Also, a different scale is used in range 900 – 999 with single change in unit resulting in a great change of brightness (relatively). Such features enable this instrument to be used in diverse situations without suffering from any technical limitation. In future we want to replace mechanical rotation of pictures into electronically controlled series of pictures.

implications:-

The purpose of this study to evaluate &  Standardize the “Dark Adaptometer”  with aim to develop an efficient evaluation method for vitamin A deficiency & encourage its use in various fields to form efficient, low cost, less time consuming screening method & long term health benefits

Problems in detection: Where there is electricity, it is difficult to detect vitamin A deficiency due to lighting of electric bulb. Where there is no electricity – mostly too remote areas – people are so habituated with existing deficiency of Vitmain A ( they come back from work before sun set ) that they take it as granted that all people in world will have these conditions & all phenomenon which we label as abnormal is accepted as  normal.

The detection based on blood level will require modern  laboratory & taking sample of blood.  Each individual may have to spend around Rs. 1000 to 2000 for getting the blood level of vitamin A.

The instrument:

The instrument works on a simple principle of objectively measuring ability of an individual to identify the picture in a hollow tube in standardized illumination. (Standardization is to be done in near by area  in relatively healthy population having rich diet in vitamin A  or it is to be adopted based on findings of other area.) The diagnosis can be confirmed after testing the vision after giving vitamin A in appropriate dosages. (After a week & 2 weeks interval of vitamin A administration.)

 

The instrument is ready for testing purpose. At present it is undergoing minor modification to make sturdy in field situation.  Various version were tried till date in drivers , school children , in general population in exhibition & at times there was correlation with conjuctival  xerosis , bitot’s  spot etc.  It has taken two decades for various trials & errors in finalizing the design of the instrument.
